Effects of an Angiotensin-Converting Enzyme Inhibitor on Residual Renal Function in Patients Receiving Peritoneal Dialysis. A Randomized, Controlled Study PK-T. Li, K.-M. Chow, T.Y.-H. Wong, C.-B. Leung, and C.-C. Szeto
Section : Articles
Areas of interest : Nephrology.
The main conclusion of this study is that ramipril, an angiotensin-converting enzyme inhibitor, can reduce the loss of kidney function in patients with advanced kidney failure and who are undergoing peritoneal dialysis.

Statin Use, Clinical Fracture, and Bone Density in Postmenopausal Women: Results from the Womens Health Initiative Observational Study AZ LaCroix, JA Cauley, M. Pettinger, J. Hsia, DC Bauer, J. McGowan, Z. Chen, CE Lewis, SG McNeeley, MD Passaro, and RD Jackson
Section : Articles
Areas of interest: Osteoarticular pathology. Osteoporosis. Family medicine. Internal Medicine. Rheumatology. Osteoporosis.
The data come from the Women’s Health Initiative Observational Study, which includes 93,716 postmenopausal women between 50 and 79 years of age. The study results do not support the recommendation of the use of statins as an effective drug to prevent or treat osteoporosis. The risk of hip fracture was 1.22 (95% CI, 0.83 to 1.81), a result without statistical significance.

Effect of Losartan on Microalbuminuria in Normotensive Patients with Type 2 Diabetes Mellitus. A Randomized Clinical Trial AAM Zandbergen, MGA Baggen, SWJ Lamberts, AH Bootsma, D. de Zeeuw, and RJTh. Ouwendijk
Section : Articles
Areas of interest : Diabetes. Nephrology. Hypertension. Family medicine. Internal Medicine. Nephrology. Diabetes. Arterial hypertension.
The conclusion of this 10-week randomized clinical trial is that losartan, an angiotensin receptor antagonist, reduces urinary albumin excretion by 34% in normotensive type II diabetic patients with microalbuminuria. Furthermore, the effect of losartan on proteinuria is independent of the effect on blood pressure.

Alternation of Antiretroviral Drug Regimens for HIV Infection. A Randomized, Controlled Trial  J. Martinez-Picado, E. Negredo, L. Ruiz, A. Shintani, CR Fumaz, C. Zala, P. Domingo, J. Vilaró, JM Llibre, P. Viciana, K. Hertogs, C Boucher, RT DAquila, B. Clotet, and the SWATCH Study Team
Section : Articles
Areas of interest : Internal medicine. Microbiology and parasitology. AIDS.
In this clinical trial, patients were assigned to receive one of two three-drug regimens (stavudine, didanosine and efavirenz or lamivudine, nelfinavir and zidovudine) or to receive the two treatments alternately every three months. With this last option it was possible to delay virological failure (incidence rate, 1.2 events/1000 person-week [95%CI, 0.3 to 3.6 events/1000 person-week] versus 4.8 events/1000 person-week [CI , 2.9a 7.4 events/1000 person-week]; P = 0.01).

Diagnosis and Management of Adults with Pharyngitis. A Cost-Effectiveness Analysis  JM Neuner, MB Hamel, RS Phillips, K. Bona, and MD Aronson
Section : Academia and Clinic
Areas of interest : Infectious Diseases. Family medicine. Internal Medicine. Microbiology and parasitology.
Observation, culture, and the application of a rapid diagnostic test for the detection and treatment of pharyngitis due to group A beta-hemolytic streptococcus in adults have a very similar cost-effectiveness ratio. When the probability of developing pharyngitis due to group A beta-hemolytic streptococcus is equal to or greater than 10%, the lowest cost strategy is culture. Empirical antibiotic treatment is never the best strategy, except in patients with a very high probability of suffering from group A beta-hemolytic streptococcal pharyngitis.
